Misophonia: Clinical Understanding and Treatment with the EASE Model

Misophonia is an underrecognized condition in which everyday sensory triggers—such as chewing, breathing, or tapping—elicit disproportionate emotional responses ranging from irritation to rage. While often dismissed as a simple sensitivity, misophonia has profound implications for emotional regulation, relational functioning, and overall quality of life. Clinicians are increasingly encountering patients who describe these struggles but lack a clear diagnostic or treatment framework.

In this seminar, Dr. Ezra Cowan will present a clinically grounded overview of misophonia, highlighting its developmental and emotional underpinnings, its frequent links with perfectionism and rigidity, and the diverse spectrum of triggers beyond the “classic” sounds. He will then introduce the EASE Model, a structured therapeutic approach tailored for misophonia:

External Acceptance — fostering cognitive and behavioral flexibility by helping patients relinquish the pursuit of external change.

Self Engagement — cultivating emotional flexibility through compassionate inward focus and the capacity to tolerate difficult internal states.

Drawing on clinical case material, empirical findings, and practical interventions, Dr. Cowan will illustrate how the EASE Model can be implemented in therapy. Participants will gain a deeper understanding of misophonia as well as concrete strategies for supporting patients in reducing avoidance, processing their emotional responses, and reclaiming fuller engagement with life.

About the Presenter

Ezra N. Cowan, PsyD is the Founder and Clinical Director of the Center for OCD and Misophonia, where he provides specialized care for obsessive-compulsive disorder (OCD), perfectionism, and misophonia. He is a nationally recognized expert on misophonia, having published a theoretical model (the EASE Model) and contributed original clinical insights into its developmental and emotional underpinnings.


Dr. Cowan regularly presents at professional conferences and seminars, and has served as an expert witness. His clinical and scholarly work has helped establish greater recognition of misophonia as a legitimate mental health condition, and his treatment model has guided both clinicians and patients toward practical, research-informed pathways of relief.
