This course introduces clinicians to several widely used—and meaningfully different—treatment modalities in contemporary psychosexual therapy. Emphasis is placed on how each model conceptualizes sexual distress, what mechanisms of change it prioritizes, and how a clinician decides which approach fits a given presentation. Students will learn to differentiate relational-growth models (e.g., Schnarch’s Crucible Model), couple-centered satisfaction frameworks (e.g., the Good Enough Sex Model), systems-based formulations (Systemic Sex Therapy), structured skills-based interventions (Cognitive-Behavioral Sex Therapy), and depth-oriented meaning-making approaches (Existential–Experiential and Narrative Sex Therapy). The course also covers clinically responsible integration of pharmacologic options into a biopsychosocial treatment plan, including medication review, collaboration with prescribers, and the ethical boundaries of pharmacologic strategies in higher-risk presentations. Throughout, learners will translate theory into clinical decision-making using case formulation, modality selection, pacing, contraindications, and relapse-prevention thinking—so that treatment is not merely “technique-driven,” but strategically matched to the client/couple, the context, and the clinical problem.

About the Presenter

Caleb Jacobson is an internationally recognized clinical psychologist, sex therapist, and Bible scholar. He holds a Doctor of Psychology (PsyD) degree in clinical psychology and a Doctor of Philosophy (PhD) in Hebrew Bible, biblical archaeology, and psychological exegesis. In his clinical practice, Dr. Jacobson specializes in working with religious and conservative patients, treating male sexual dysfunction, and addressing rare and problematic sexual issues. A pioneer in the integration of sex therapy and religion, he is the author of the award-winning Sex Therapy with Religious Patients: Working with Jewish, Christian, and Muslim Communities (2024) and the editor ofThe Routledge International Handbook of Sex Therapy and Religion (2025). Dr. Jacobson currently serves as president of both the School of Sex Therapy and the International Association of Psychosexual Therapists, where he leads initiatives in education, research, and clinical advancement within the field.

Learning Objectives:

  1. Compare major psychosexual therapy modalities by articulating each model’s core assumptions, mechanism(s) of change, and primary clinical targets (desire, arousal, orgasm, pain, intimacy, attachment, meaning, and sexual scripts).
  2. Formulate sexual concerns using model-specific frameworks (e.g., differentiation and gridlock; “good enough” variability; Intersystem domains; cognitive-affective/performance anxiety; attachment cycles; problem-saturated narratives; existential meaning and embodiment).
  3. Select and justify a modality (or integrative sequence) based on presenting problem, readiness, relational context, risk factors, and contraindications (e.g., coercion, ongoing violence, untreated trauma, severe instability).
  4. Apply modality-consistent interventions at a basic level (e.g., structured CBT-based exercises; systemic multi-level treatment planning; narrative externalizing and re-authoring; attachment-based de-escalation and bonding conversations; experiential presence and meaning-focused exploration) while maintaining appropriate pacing and clinical safety.
