"Working with Out of Control Sexual Behavior", provides a comprehensive and integrative clinical framework for understanding and addressing problematic sexual behaviors, especially those popularly conceptualized as sex addiction. It critically examines mainstream diagnostic labels, the cultural influence on sexual norms, and the limitations of the addiction and disease model. Drawing on sexological perspectives and harm reduction principles, the class will explore alternative models such as Compulsive Sexual Behavior Disorder (CSBD) and Moral Incongruence, and place strong emphasis on individualized assessment, clinical neutrality, and value-sensitive treatment. The perspective presented integrates psychoanalytic, relational, and trauma-informed approaches, advocating for therapy that accepts diverse expressions of sexuality, supports autonomy, and attends to relationship and attachment dynamics.

About the Presenter

Wendy E. Miller, PhD, is a psychologist, psychoanalyst, and an AASECT-certified sex therapist (CST) with 40 years of experience working with individuals and couples. She is a frequent presenter on OCSB and other sex therapy topics.

Learning Objectives:

  1. Describe the process of conducting a comprehensive biopsychosocial assessment for individuals presenting with problematic sexual behaviors.
  2. Name multiple functions of action symptoms( eg. compulsive sexual behavior) as meaningful communications that may express and help manage affect, trauma, or relational conflict, emphasizing the importance of mentalization and integration over symptom eradication.
  3. Identify specific strategies to address ambivalence, shame, and avoidance in clinical work with OCSB, including group therapy, mindfulness techniques and microanalysis of behavior patterns to promote durable change and deeper self-understanding.
